Question:

I am currently 70% PTSD.  I just had a heart attack and a pace/ defibrillator placed in me. Should I file for my heart and if so,  in connection with PTSD or by itself?

I'll assume you're a Vietnam veteran so the heart attack will be tied to agent orange exposure and the defib will be secondary to that. Yes, you should immediately file a claim for the heart attack and the defib. Don't worry about the rest, get those claims filed today!
