Question:

I review VA Form 21-526EZ and instructions on how to complete this form and nowhere did it say that DBQ forms had to be submitted. I thought a DBQ had to be submitted with each claim.

No, there is no requirement to submit a DBQ with your claim. If you have a DBQ prepared by a physician who knows you, that will probably help your case. Otherwise a DBQ will be completed for you during a C & P exam.
