Should I use QTC for my C & P exam?  Or should I get the forms for my private doctor to complete for me to send to the VA for their C & P exam?

That isn't a choice that VA offers to you. When ordered to attend a C & P exam you must go and allow the VA appointed examiner to complete the task. To avoid a C & P exam whether by a contractor or a VA staffer is to default to a loss of your claim. You're encouraged to submit your own medical evidence as an addition to the findings of the C & P examiner. VA takes their C & P exams very seriously and you should too.
